For breed certification exams, clients must complete the appropriate paperwork through the Orthopedic Foundation for Animals (OFA). You can begin your application online before your visit.

How to Complete the OFA Form Online Before Your Visit

  1. Go to www.OFA.org
  2. Click “Log In” in the top right corner
  3. Log in to your existing account or create a new one
  4. Select the blue “Create a New Application” button
  5. Under “Animal Information”:
    • Search for your pet if they have had an OFA exam before
    • Or complete all required fields with your pet’s information
  6. Under “Exam/Service,” choose “Online Exams”
  7. In the “CAER Eye Exam” box, select “In a Veterinary Office”
  8. In the search box, type “Animal Eye Specialists”
  9. Select Dr. Stephanie Beaumont from the list

Once complete, select Save (or Save + New if adding additional animals).

On the day of your visit, we will access the form in our system and complete the exam findings. After we finalize our portion, you will receive an email notification from OFA. Please note that you must submit payment directly to OFA in order to officially process and finalize the certification.
